<description>De la inconsistencia a la Estadística plausible en la investigación médica una aplicación a la homeostasis pancreática y metabólica tras la cirugía bariátrica
Almorza Gomar, David
Este trabajo se presenta dentro del modelo de Tesis como compendio de publicaciones, e incluye&#13;
tres artículos publicados entre el mes de noviembre de 2022 hasta la actualidad. Se trata de&#13;
artículos JCR de primer, segundo y tercer cuartil.&#13;
Todos los artículos tratan sobre nuestra investigación en cirugía bariátrica y sus efectos en&#13;
cuanto a los cambios que sufre el páncreas en su fisiología y en su funcionalidad, así como&#13;
las consecuencias entre el tipo de cirugía llevada a cabo. A raíz de esto, se analizan las&#13;
consecuencias metabólicas y fisiopatológicas de las cirugías.&#13;
Con carácter especial se dedica mayor énfasis al tratamiento estadístico de los datos utilizados&#13;
en las investigaciones del ámbito biomédico y de los errores que con mayor frecuencia se&#13;
producen en la bibliografía internacional aceptada (sobre todo en cuanto al uso del error&#13;
estándar de la media (SEM), de la variabilidad o el cálculo del área bajo la curva (AUC)).&#13;
Como resumen de las conclusiones alcanzadas: se realizan nuevos métodos quirúrgicos&#13;
experimentales, observándose las compensaciones tempranas y tardías en la secreción de&#13;
insulina y los mecanismos que no solo resuelven, sino que condicionan el fracaso de estas&#13;
estructuras. Observamos el comportamiento de las distintas porciones del tubo digestivo en&#13;
sus componentes absortivo y secretor endocrino. El trabajo finaliza con un protocolo&#13;
completo para considerar el tratamiento correcto de los datos en este tipo de investigaciones&#13;
y la observación de estadística correcta de los mismos.; This work is presented within the Thesis model as a compendium of publications, and includes three&#13;
papers published from the month of November 2022 to the present. They are JCE papers&#13;
of first, second and third quartiles.&#13;
All papers deal about our research with bariatric surgery and its effects in terms of the&#13;
changes that the pancreas undergoes in its physiology and functionality, as well as the&#13;
consequences among the type of surgery carried out. Following this, the metabolic and&#13;
pathophysiological consequences of surgeries are analysed.&#13;
In particular, greater emphasis is placed on the statistical treatment of data used in biomedical&#13;
research and on the errors that most frequently occur in the accepted international&#13;
bibliography (especially regarding the use of the standard error of the mean (SEM), of the&#13;
variability or the calculation of the area under the curve (AUC)).&#13;
As a summary of the conclusions reached: new experimental surgical methods are carried&#13;
out, observing early and late compensations in insulin secretion and the mechanisms that not&#13;
only resolve, but also determine, the failure of these structures. We observe the behaviour of&#13;
the different portions of the digestive tract in these absorptive and endocrine secretory&#13;
components. The work ends with a complete protocol to consider the correct treatment of&#13;
the data in this type of research and the observation of correct statistics thereof.

La cirugía bariátrica/metabólica ha llegado a ser una potente herramienta no solo&#13;
contra la obesidad, sino también frente a algunas de sus principales complicaciones, como&#13;
la diabetes mellitus tipo 2, que en algunos casos presenta resultados superiores al&#13;
tratamiento farmacológico tradicional. La hipoglucemia postprandial o efecto dumping,&#13;
constituye una complicación relevante que presenta una de las técnicas quirúrgicas más&#13;
llevada a cabo por sus buenos resultados y por una técnica quirúrgica que no se excede&#13;
en complejidad. Este fenómeno se produce por una acelerada absorción de glucosa en el&#13;
intestino delgado proximal y una respuesta insulínica exagerada.&#13;
En este estudio se utiliza un inhibidor parcial del co-transportador sodio-glucosaSGLT-1/2, como canagliflozina, y con ello evaluar la capacidad de revertir estos episodios&#13;
de hipoglucemia postpandrial, en un modelo de roedor sano sometido a gastrectomía&#13;
vertical, comprobando que el modelo no altera los beneficios metabólicos de los cambios&#13;
enterohormonales característicos del reordenamiento anatómico postquirúrgico. Los&#13;
resultados obtenidos indican que la inhibición parcial de SGLT-1 limita el excesivo flujo&#13;
glucídico desde la luz intestinal hacia la circulación, atenuando la secreción insulínica&#13;
abrupta y reduciendo la incidencia de hipoglucemia postprandial, sin modificar los&#13;
patrones hormonales derivados de la cirugía. En conjunto, estos hallazgos respaldan la&#13;
hipótesis. El fármaco canagliflozina puede modular selectivamente la absorción de&#13;
glucosa preservando la fisiología enteroendocrina inducida por la gastrectomía vertical,&#13;
y sugiere una potencial indicación terapéutica, como estrategia para el manejo del&#13;
síndrome de dumping.; Bariatric/metabolic surgery has become a powerful tool not only in the fight&#13;
against obesity but also in addressing some of it is main complications, such as type 2&#13;
diabetes mellitus, in which surgical intervention has, in certain cases, shown superior&#13;
outcomes compared to traditional pharmacological treatment. Postprandial&#13;
hypoglycemia, or dumping syndrome, is a significant complication associated with one&#13;
of the most commonly performed surgical techniques, due to it is favourable outcomes&#13;
and moderate surgical complexity. This phenomenon results from accelerated glucose&#13;
absorption in the proximal small intestine and an exaggerated insulin response.&#13;
In this study, a partial inhibitor of the sodium-glucose co-transporter 1/2 (SGLT1/2), such as canagliflozin, was used to evaluate its ability to reverse episodes of&#13;
postprandial hypoglycemia in a healthy rodent model subjected to vertical sleeve&#13;
gastrectomy. It was confirmed that the model did not alter the metabolic benefits arising&#13;
from enteroendocrine changes induced by post-surgical anatomical rearrangement. The&#13;
results indicate that partial inhibition of SGLT-1 limits the excessive glucose influx from&#13;
the intestinal lumen into the circulation, thereby attenuating the abrupt insulin secretion&#13;
and reducing the postprandial hypoglycemia, without altering the hormonal patterns&#13;
derived from the surgery. Collectively, these findings support the hypothesis that&#13;
canagliflozin can selectively modulate glucose absorption while preserving the&#13;
enteroendocrine physiology induced by vertical sleeve gastrectomy, suggesting it is a&#13;
potential therapeutic use as a strategy for the management of dumping syndrome.

<title>Cell replacement strategies in the central nervous system in two mouse models of neuronal damage: potential therapeutic targets and promising drugs</title>

<description>Cell replacement strategies in the central nervous system in two mouse models of neuronal damage: potential therapeutic targets and promising drugs
Gómez Oliva, Ricardo
The recent discovery of adult neural stem cells (NSC) has revealed the therapeutic potential of neurogenesis in the development of new regenerative therapies to treat the damaged adult brain. Physiologically, the generation of new neurons from NSC occurs mainly within two regions of the adult brain: the subventricular zone (SVZ) and the dentate gyrus (DG) of the hippocampus. However, the homeostatic mechanisms that lead to the generation of neurons can be modified under different pathological circumstances. We have studied in here two pathological conditions that affect neurogenesis in the SVZ and DG.&#13;
&#13;
In response to cortical brain injuries, neurogenesis is stimulated in the SVZ, however no newly generated neurons can be found around the injury. Despite the attempt of neuroblasts to migrate toward the injury, they fail to cross the corpus callosum and reach the perilesional area. We have aimed in this thesis to understand the mechanisms impairing migration. Our results indicate that in response to an injury, microglial cells activated within the injury and the SVZ release TGF¿, activating the EGFR expressed in the neuroblasts membrane inducing their proliferation, delaying maturation and negatively regulating migration. The inactivation of this signaling pathway with the EGFR inhibitor Afatinib stimulates neuroblast migration toward the injury and leads to neuroblast enrichment within the injured area.&#13;
&#13;
Likewise, neuropathological aging, is associated to cognitive decline and to alterations in neurogenesis in the DG. Neuropathology in the aged mouse brain alters the progeny of NSC leading to a larger number of astrocytes and to aberrant newly generated neurons. We have tested in here the effect of the small molecule ER272 in a murine model of neuropathological aging, the SAMP8 mouse. We show that in six-month-old SAMP8 mice, episodic and spatial memory are impaired, a reduced number of newly generated neurons is found, which show an aberrant morphology, and the generation of astrocytes is increased in this model. The treatment of SAMP8 mice with ER272 prevents these defects, positively regulating neurogenesis and improving cognitive performance. We have tried to elucidate the cellular mechanisms involved in its effect.&#13;
&#13;
Finally, vitamin D deficiency has been shown to be an accelerating factor for the progression of the aging process, leading to cognitive impairment. We have reviewed the role that the Wnt signaling pathway plays in neurogenesis in the aged mouse brain and have analyzed the relation between vitamin D and Wnt signaling to hypothesize that vitamin D may play a role in positively regulating neurogenesis through Wnt signaling and maintaining cognitive performance.

La población obesa se espera que continue creciendo en los años venideros. La cirugía barbárica-metabólica ha demostrado un buen control sobre el peso, pero más considerablemente sobre las comorbilidades asociadas como la diabetes o hipertensión.&#13;
&#13;
La razón por lo que esto ocurre de forma independiente a la pérdida ponderal continua siendo objeto de estudio. Varios autores apuntan a la importancia de la microbiota o bien factores mecánicos como la restricción gástrica o la malabsorción de nutrientes. Una gran mayoría le otorga un papel fundamental a las incriminas, que actúan como catalizadores de funciones específicas del tubo digestivo (teorías del intestino proximal y distal).&#13;
&#13;
En base a las técnicas quirúrgicas llevadas a cabo en la práctica clínica diaria, ambas teorías son excluyentes entre sí.&#13;
&#13;
