Abstract

Introduction: Postoperative pulmonary complications (PPC) have an incidence of 30-40% in cardiac surgery, and are associated with increased morbidity and mortality. Postoperative respiratory physiotherapy protocols have been shown to be effective in the short term; however, there are currently few studies investigating their effect when performed prior to surgery. Hypothesis: The introduction of preoperative respiratory physiotherapy exercises will reduce the incidence of pulmonary complications after cardiac surgery. Objective: To determine whether preoperative respiratory physiotherapy reduces the incidence of PPC in cardiac surgery patients. Materials and methods: Experimental, longitudinal, prospective, randomised, randomised, controlled and open study that evaluated the introduction of a preoperative respiratory physiotherapy programme with spirometer (FISIO group) versus the usual protocol (CONTROL group). The data were collected from the Diraya and SICCS programmes. Results: The CONTROL and PHYSIO groups had 20 patients each (total n=40). In the bivariate analysis, significance was found in the main variable PPC (50% CONTROL, 20% PHYSIO; p=0.047) and intubation time (9.5 hours CONTROL, 8 hours PHYSIO; p=0.025), although in the logistic regression the relationship between PPC and PHYSIO lost significance (p=0.1). The variables number of PPCs, types of PPCs, time in ICU and on the ward had no significant differences between the two groups. Conclusion: Strengthening the respiratory musculature with physiotherapy in the preoperative period is a potential protective factor against PPCs, as well as reducing intubation time. However, the lack of consensus on the protocol in the various studies makes it difficult to demonstrate its validity.
