Abstract

Bariatric/metabolic surgery has become a powerful tool not only in the fight against obesity but also in addressing some of it is main complications, such as type 2 diabetes mellitus, in which surgical intervention has, in certain cases, shown superior outcomes compared to traditional pharmacological treatment. Postprandial hypoglycemia, or dumping syndrome, is a significant complication associated with one of the most commonly performed surgical techniques, due to it is favourable outcomes and moderate surgical complexity. This phenomenon results from accelerated glucose absorption in the proximal small intestine and an exaggerated insulin response. In this study, a partial inhibitor of the sodium-glucose co-transporter 1/2 (SGLT1/2), such as canagliflozin, was used to evaluate its ability to reverse episodes of postprandial hypoglycemia in a healthy rodent model subjected to vertical sleeve gastrectomy. It was confirmed that the model did not alter the metabolic benefits arising from enteroendocrine changes induced by post-surgical anatomical rearrangement. The results indicate that partial inhibition of SGLT-1 limits the excessive glucose influx from the intestinal lumen into the circulation, thereby attenuating the abrupt insulin secretion and reducing the postprandial hypoglycemia, without altering the hormonal patterns derived from the surgery. Collectively, these findings support the hypothesis that canagliflozin can selectively modulate glucose absorption while preserving the enteroendocrine physiology induced by vertical sleeve gastrectomy, suggesting it is a potential therapeutic use as a strategy for the management of dumping syndrome.
